When the mask just doesn't work: what now?

If you’ve given CPAP a real shot—but the mask, the pressure, or the whole setup just doesn’t work for you—you’re not alone.

At Whitney Sleep Center, we regularly hear from patients who feel frustrated, defeated, or even ashamed after struggling with CPAP.

The truth? CPAP is effective, but it’s not for everyone. And that doesn’t mean you’re out of options. There are safe, medically supported alternatives that can help manage your sleep apnea without a mask.

Why some people can't tolerate CPAP

Even with proper instruction and equipment, up to 50% of users don’t stick with CPAP long term. Common reasons include:

  • Air leaks or skin irritation from a poor-fitting mask
  • Claustrophobia or anxiety, especially in trauma-sensitive patients
  • Trouble exhaling against pressure (“air hunger”)
  • Nasal congestion that blocks airflow through the mask
  • Noise, bulk, or discomfort—especially for light sleepers or partners

If adjustments like switching masks or adding humidification haven’t helped, it’s time to explore what else can work.

Talk to your provider: personalization matters

No single treatment fits everyone. That’s why the team at Whitney Sleep Center works closely with each patient to:

  • Evaluate OSA severity and symptom patterns
  • Identify causes of CPAP intolerance
  • Match treatments to anatomy, lifestyle, and medical conditions

In many cases, a multidisciplinary approach is ideal—combining input from a sleep physician, sleep dentist, ENT, or surgeon as needed.

And no matter which path you choose, follow-up sleep testing is crucial to ensure your treatment is working and your health risks are managed.
